Perhaps it feels like you're drowning in endless forms and meetings alone, and wish you had someone experienced by your side to guide you? Or maybe you're looking for ways to make life more enjoyable through social connections and activities, but don't know where to start.
Clients occasionally encounter challenges that extend beyond clinical issues, often requiring assistance in developing essential life skills or accessing community resources. Our Community Support Services (CCSS) bridge these gaps by providing dedicated Community Support Workers (CSWs) to provide compassionate, personalized support every step of the way.
Our Community Support Workers are committed to helping individuals and families connect with the necessary services and resources to foster resilience and achieve their goals - be it independent living, pursuing education, employment, or finding fulfilment in social and recreational activities. We offer services both in our office and out in the community, meeting you wherever you need us most.

HOW CCSS CAN HELP
What are Comprehensive Community Support Services and How Does it Work?
CCSS is a client-centered program designed to promote independence and strengthen connections with community resources, helping individuals succeed in education, employment, housing, and more.
Meetings with your Community Support Worker (CSW) will take place both at our office and in the community. The frequency and duration of these meetings will depend on your specific goals and needs.
For example, a meeting may involve CSW support during a 504 or Individualized Education Program (IEP) meeting at your child’s school, assistance with completing an application at the housing department, or help with resume creation and job searching at the library. Your CSW will maintain communication with you and your therapist to monitor progress and ensure a coordinated support team.

Frequently Asked Questions
Who can receive CCSS? CCSS is available for individuals who require additional support within the community and have Medicaid insurance coverage. Clients with a therapist outside of A New Hope are welcome to receive CCSS while continuing their therapy with their chosen provider.
What age can you receive CCSS from? Anyone from the age of 5 and upwards.
What will this process look like? After we receive your completed referral, our CSW will schedule a meeting with you to complete the intake process and identify your goals. These initial meetings will be held at our office.
Meet Our Community Support Worker (CSW)
My work in CCSS allows me to help individuals and families by helping them obtain stabilization in areas of need, such as housing, food insecurities and accessing other services. I enjoy working with schools to help identify behavioral management resources in the classroom to assist children in receiving the education they need. It is very rewarding to assist individuals in securing employment and helping prepare them to be successful with job seeking, application, obtainment and maintenance. I also enjoy helping people strengthen their social skills, conflict resolution, boundary setting and communication skills to strengthen the relationships in their lives. My goal is always to help people feel seen, heard, and valued.
